The ActiGait® system is a partially implantable medical device for treatment of dropfoot which activates the muscles of the lower leg during walking. The system is intended for use with patients suffering from paresis of the ankle dorsiflexor muscles caused by damage to the central nervous system. The largest group of patients is patients with hemiplegia as a consequence of stroke. The system is not indicated for people with dropfoot caused by peripheral nerve damage.
The ActiGait® system stimulates the peroneal nerve just proximal to the knee. This stimulation leads to a contraction of the ankle dorsiflexor muscles (m. tibialis anterior, m. extensor hallucis longus, m. extensor digitorum longus, m. peroneus tertius, m. peroneus longus and m. peroneus brevis) followed by a variable dorsiflexion of the foot. These muscles create dorsiflexion, inversion and eversion of the foot and it is therefore necessary to be able to adjust the stimulation levels of these muscles in order to produce a good balanced dorsiflexion movement suited for walking. The stimulator has therefore been developed with four independent channels, controlling four separate sets of electrodes situated inside a cuff electrode. The construction of the cuff electrode is made so that stimulation is applied at different locations around the nerve depending on which channel is active. Hereby different fascicles in the peroneal nerve can be activated inside the nerve resulting in activation of different muscles in the lower leg.
